The student or the student’s legal guardian allows DriveWise to maintain and transmit the student’s course records and other information on this form as prescribed by the MTO. 

This information shall be kept in strict confidence and shall be transmitted in accordance with MTO directives for the sole purpose and only as it pertains to the MTO regulations for approved beginner driver education programs.
